Republic of Singapore Air Force F-16C/D Crash

A Republic of Singapore Air Force F-16 became uncontrollable during takeoff from Tengah Air Base after two degraded pitch-rate gyroscopes sent matching erroneous inputs to the digital flight-control computer, according to Singapore’s Ministry of Defence. The ministry said the pilot followed emergency procedures and ejected inside the base before the aircraft crashed. Investigators found that maintenance requirements had been followed and pre-flight tests had detected no fault. The service inspected its remaining fleet and introduced periodic specialist testing of the gyro assemblies before resuming F-16 flying.
